Apply for Services
To begin the application process and be placed on the waiting list for services at Bittersweet, fill out the below digital interest form. You may also fill out a paper application and submit it via mail (12660 Archbold-Whitehouse Rd., Whitehouse, OH 43571) or via e-mail (tchambers@bittersweetfarms.org).
Once you submit your digital application, you will see a confirmation message on your screen verifying that your information has been received.

Before applying to be placed on the wait list, please note the ages eligible for the programs/services at each location. Please also note that applicants may apply before they come of age for their desired program. Early applications are highly encouraged.
Whitehouse: ICF (18+), Community Living and HUD Apartments (18+), Women’s Home (18+), Day Program (18+), Summer Enrichment* (13-22), Social Recreation Program* (13+)
Pemberville: Residential (18+), Transition Program (12-21), Summer Enrichment (12-22)
Betty’s Farm (Lima): Day Program (18+), Social Recreation Program* (13+)

Admissions & Consulting tours take place at our Whitehouse location on the 1st Wednesday of the month at 10am, May-November. Reservations are required. To reserve your spot, contact:
